“Release of Confidential HIV-related Information; Release Form”
Licensed Arizona insurers may not disclose an individual’s confidential HIV information unless the individual or their authorized representative consents to the disclosure or another law authorizes disclosure without consent. Individuals must issue their consent by signing a written release form that specifies the persons authorized to receive the information, the reason for the disclosure, the duration of the release form’s validity; such duration may not exceed 180 days.
