The Department of Public Health must, by October 1, 1989, establish regulations that “set minimum standards for hospital discharge planning services.” The standards must require (1) the physician, patient, and patient’s family or representatives to collaborate on a written discharge plan; and (2) a process for giving the patient advance notice of their discharge and a copy of their discharge before their discharge.
