“Record keeping and Reporting Shortages”
Licensed Arizona physicians must maintain prescription records and purchased records for three years after the date of dispensing the prescription or purchasing the controlled substance or prescription Physicians must immediately contact local law enforcement upon discovering the theft of loss of a controlled substance and then submit a report of the theft or loss to local law enforcement, the DEA, and the Arizona Medical Board.
