A health care provider or entity is required to report to the Board of Medicine the name of any physician whose privileges have been suspended or revoked, or who has resigned due to unprofessional conduct or clinical competence reasons within 60 days. The report must also provide relevant information, such as a description of the adverse action, the location of the action, and the patient involved. The Board may also request access to additional materials, such as medical records, written statements, and correspondence related to the incident. Failure to report can subject the health care provider or entity to a fine of $5,000.
