Initial appointment of a pregnant minor with a licensed physician from whom an abortion is sought and certification procedure for the decision-making assistance program.
Requires abortion providers to offer decision-making materials in video and written form to pregnant minors during the initial appointment for abortion services. Requires the minor to sign a form certifying that they were offered these materials and for the physician to place the certification form in the minor’s medical records.
