Physician Services Benefit Coverage Standards
Identifies the language and hearing services covered by Colorado’s Medicaid program. Specifies the information that Medicaid providers must document in client medical records (e.g., client name, services provided, referral information, plan of care, discharge summary, etc.). Defines eligible clients as those aged 20 or under and adults for who the services are medically necessary.
