Patient medical records - 28 Pa. Code § 119.24

Outpatient services in hospitals must maintain an accurate medical record for every patient; outpatient service records must be made a part of the patient’s general medical record. If a patient is transferred or referred from the outpatient service to another physician or health care facility, or is admitted to the hospital, all information necessary for his ongoing treatment must be sent promptly to the receiving physician or facility. When a patient has been referred from another agency or facility, the outpatient service must try to obtain the records and information as are necessary to assure continuity of care for that patient. The patient must authorize any transfer of records or requests to obtain records.
