Reporting of Incidents
All licensed outpatient facilities must report any serious incident or unusual occurrence regarding the safety, health, and welfare of the patients or staff to the licensing authority. This includes, but is not limited to:
- Fire, flood, or other natural disaster that creates structural damages to the facility or causes health hazards;
- Serious outbreaks of contagious diseases dangerous to public health;
- Serious human errors by staff members of the facility that result in the death, serious illness, or physical impairment of a patient.
