Anyone who suspects that a resident of a long term care facility has been the subject of abuse must immediately report the suspected abuse to the local office of the area agency on aging, the Department of Human Services or to a law enforcement agency within the county where the person making the report is at the time of contact. The report should contain the names and addresses of the resident, the nature and extent of the abuse, the explanation given for the abuse and any other information which the person making the report believes might be helpful in establishing the cause of the abuse and the identity of the perpetrator.
