Adult Foster Homes must comply with a number of standards for care and services, including the following:
- Maintain a copy of medication, treatment and therapy orders in the resident’s file for the use of any medications, treatments and other therapies;
- Keep individual records for each admitted resident that includes the resident’s name, previous address, date of entry, date of birth, sex, marital status, religious preference, preferred hospital, Medicaid and/or Medicare numbers if applicable, and guardian status;
- Make resident records available to representatives of the Department of Mental Health Services conducting inspections or investigations
- Make resident records available to residents and their authorized representative or other legally authorized persons;
- Retain resident records for three years after discharge;
- Make separate written incident reports of all unusual incidents relating to the Adult Foster Home, including to resident care, for each resident involved
- Submit written incident reports to the Community Mental Health Program within five days of the incident; any incident that is the result of or suspect of abuse must be reported to the Office of Investigations and Training within 24 hours of occurrence;
- Allow the Department of Mental Health Services personnel or designee access to all records; and
- Keep information related to the resident confidential.
