Individuals required to report suspected abuse of mentally ill or developmentally disabled persons must report the name, age and present location of the allegedly abused adult, the nature and extent of the alleged abuse, any information that led the person making the report to suspect that abuse has occurred and any other information that the person believes might be helpful in establishing the cause of the abuse and the identity of the perpetrator. Upon receipt of a report of abuse, the department or its designee must notify the agency providing primary case management services to the adult and the guardian or case manager of the adult, unless the guardian or case manager is suspected of committing abuse.
