Within one week of dispensing a controlled substance to a Division of Medical Assistance Program fee-for-service client, pharmacies must electronically report to the Division the following information:
- Patient's full name, address, and date of birth;
- Pharmacy Drug Enforcement Administration Registration Number;
- Prescriber name and Drug Enforcement Administration Registration Number;
- Identification of the controlled substance using a national drug code number;
- Date the prescription was written;
- Date the drug was dispensed; and
- Number of metric units dispensed.
