A Primary Care Manager (PCM) is a health care provider that agrees to provide primary care management services to an Oregon Health Plan client enrolled in the PCM Program. The PCM must maintain a medical record keeping system that is adequate to fully disclose and document the medical condition of the client and the extent of covered services and/or case-managed services received by the client. Medical records must be maintained for at least four years after the date of medical services. PCM’s ensure the maintenance and confidentiality of medical record information and may release such information to the extent permitted by the PCM’s agreement with the Division of Medical Assistance Programs (DMAP).
