Medical Records; Definitions
These requirements apply to any person or facility lawfully providing health care services in New Hampshire.
All medical information contained in the medical records in the possession of any health care provider is the property of the patient. The patient has the right to receive a copy of such records upon request, and may be charged up to $15 for the first thirty pages of the copy or $.50 per page, whichever is greater. Copies of filmed records (such as radiograms, x-rays, and sonograms) must be copied at a reasonable cost.
