Hospitals must annually report the following information:
- Its financial relationships with physician hospital organizations;
- The number and type of providers employed by the hospital and its affiliates;
- A description of the type of services provided by the hospital and its affiliates; and
- A list of physician practices that are owned by the hospital and its affiliates.
