Each nursing home, acute care hospital, residential care facility, specialty hospital, or other licensed health care facility must file the following information with the Commissioner of the Department of Health and Human Services:
- Financial information such as costs of operation, revenues, and aggregate wage and salary data;
- Ownership information;
- Number and type of beds;
- For hospitals, the data collected through the uniform hospital discharge data set and annual hospital survey;
- For facilities where the average length of stay is longer than 30 days, a uniform long-term care data set based on annual patient census, annual aggregated use data on patient days, and the discharges and admissions of the facility;
- Disposition destination of each admitted patient;
- Charge data: For acute care - charge by discharge; For facilities where average length of stay is over 30 days - average patient day charge;
- Any necessary demographic or diagnostic information.
