“Medications services under the division of public health services regulations”

All nursing home facilities must maintain all records for each medication taken by a resident that contains the following information: any allergies; the medication name, strength, dose, frequency, and route of administration; the date and time the medication was taken; the signature, initials, and job title of the person who administers the medication; and documented reason for any medication refusal or admission.
