Admission Record
A medical record must be created for each patient within 72 hours of admission. The medical record must have identifying information of each patient, such as name, previous address, social security number, age, sex, marital status, date and place of birth, occupation, time and date of admission, name and address of nearest relative, emergency contact, funeral arrangements, and the name of the patient’s physician. At the time of discharge or death, the medical record should be completed with the relevant information such as date and time of discharge, reason for discharge, and/or date and time of death.
