The Department of Community Health (“department”) must create and maintain the “Michigan care improvement registry” (“registry”) for the purpose of recording childhood immunization information. The department must comply with confidentiality and disclosure requirements while maintaining the registry. Individuals aged 20 years or older may make their personal immunization information housed in the registry inaccessible by submitting a written request to the department.
