A qualified hospital patient safety organization (one that was incorporated before Jan. 1, 2009 with a membership of at least 75% of all hospitals) must create a confidential reporting system to collect data regarding serious, adverse events that occur in hospitals in order to improve patient safety and increase the delivery of safe care. The qualified hospital patient safety organization must put out and distribute an annual report to improve patient safety in hospitals.
