Upon issuing a pharmaceutical-grade cannabis registration card to a patient, the department must enter the following information into the law enforcement information network:
- The card’s registration number;
- The patient’s name and address;
- The registration card’s expiration date; and
- The name and address of the attending physician.
While the above information must be entered into the information network, the department may not enter any information related to the diagnosis or other medical information of the patient. Such information remains confidential.
