All physicians, dentists, or other health care professionals who have knowledge of a case of cancer must report the case to the Michigan Department of Health within 10 days. The report must include the following information:
- Patient’s name
- Patient’s address
- Identity of the person reporting
- Other relevant information
All reported cases to the Department must remain confidential.
