All medication errors and reactions to medications that occur in nursing facilities must be reported to an applicable nurse, physician, or pharmacist, recorded in the patient’s record, and an appropriate corrective action must be “initiated promptly.” Additionally, an incident form must be completed and sent to the administrator.
