Confidentiality of reports
The Department of Community Health Community Public Health Agency must maintain the confidentiality of all health information received from providers and may not release any information that identifies a particular individual. The Department may only release individually identifiable immunization reports to the particular individual to his or her parent, legal guardian, or attorney, on written request. The written request must be signed by the relevant individual and must be witnessed or notarized. The individual must also provide adequate identification. Information from the Registry may be provided to a registered user upon request. The Department can release health information to an authorized representative of a study, but cannot release information obtained pursuant to MI ADC R. 325.164 (providing medical records to the Department for quality assurance).
