A health care provider who administers an immunization to a child born after December 31, 1993 or is under 20 years old, must report the immunization to the Department of Community Health within 72 hours of giving the immunization. The health care provider must be legally authorized to administer the immunization to fulfill the reporting requirements. A health care provider who recieves a request from parents to not report the immunization must submit the request to the Department.
