When requesting a blood lead analysis from a clinical laboratory, the physician must submit the appropriate information the Department. The information must include the patient’s name, sex, age, birthdate, address, and other personal information. Upon receiving the blood sample for the lead analysis, the clinical laboratory must also report the results of the blood test to the Department.
