Medical facility reports and notifications - 40 Pa. Stat. Ann. § 1303.313

Medical facilities must report to the department of health and the patient safety authority an occurrence of a serious event involving the clinical care of a patient, which result in the death of a patient or the compromise of patient safety resulting in an unanticipated injury requiring the delivery of additional health care services to the patient within 24 hours of the facility’s confirmation of the occurrence of the event. The report may not include the name of the patient or other identifiable individual information. If a medical facility discovers that a provider in the facility failed to report a serious event, the facility must notify the appropriate licensing board. Failure to comply with patient safety requirements may subject the facility to an administrative penalty of $1,000 per day, in addition to other penalties imposed by other regulations.
