Reporting and Collecting Medical Aid-In-Dying Medication Information
Specifies the information that health care providers must report within 30 days of prescribing medical aid-in-dying medication to a qualified patient (e.g., patient’s name, disease diagnosis, prognosis, etc.). Specifies the information that health care providers must report within 10 days of dispensing medical aid-in-dying medication (e.g., patients name, dispensed medication, etc.). Classifies all reported information as confidential.
