Specifies the information that an attending physician must document in a patient’s medical record prior to providing medical aid-in-dying medication. Such information includes the dates of the patient’s requests for such medication, the physician’s diagnosis and determination that the patient has the mental capacity to request the medication and make an informed decision, and the consulting physician’s determination of these factors. Requires the Department of Public Health and Environment to annually review medical records to assess compliance with the record keeping requirements. Clarifies that information obtained by the Department for this purpose is not public record and may not be disclosed, but permits disclosure of statistical information. Requires providers to file a copy of a medical aid-in-dying medication dispensing record with the Department.
