A Children's Medical Services Program participating hospital must provide the following services:
- Directing the development of the care management plan;
- Care planning and treatment;
- Coordination with others involved in the development of the care management plan;
- Discharge planning procedures;
- Parental counseling regarding a child's medical and social needs after the child’s release; and
- A discharge summary.
