Provider agreements between the Ohio department of job and family services and a provider of Medicaid covered services will contain the following provisions:
- To render medical services as medically necessary without regard to race, creed, color, age, sex, national origin, source of payment, or handicap;
- To maintain all records necessary and in such form so as to fully disclose the extent of services provided and significant business transactions for at least six years from the date of the receipt of payment based on these records or until any audit initiated within the six year period is completed;
- To furnish to the department, the secretary of the department of health and human services, or the Ohio Medicaid fraud control unit or their designees any information maintained in records for audit and review purposes; and
- To comply with the confidentiality safeguards and the use and release of information regarding public assistance recipients.
