An attending health care provider must report to the health commissioner of relevant jurisdiction the name, age, sex, race and address of a patient suffering from any of the following:
- Cholera;
- Plague;
- Yellow fever;
- Typhus fever;
- Diphtheria;
- Typhoid fever;
- Any disease dangerous to the public health; and
- Any disease required by the department of health to be reported.
Any reported information that is protected health information may only be released in accordance with relevant regulations. Information that does not identify an individual may be released in summary, statistical, or aggregate form.
