A laboratory that examines human specimens for evidence of reportable diseases must report all positive results of such examination as required. The laboratory report must include the following information:
- The case’s information, including name, date of birth, sex, and street address;
- Laboratory test information, including specimen identification number, specimen collection date, specimen type, test name, test result, and if applicable, the organism and serotype;
- The health care provider’s information, including name, telephone number, and street address.
The city or general health district in which the suspect case resides must make an inquiry through the appropriate health care provider to determine if the suspected case exists.
