"Program; program contractors; interdisciplinary teams"
Arizona establishes the "comprehensive care for the elderly program" ("program") as an alternative to the long-term care system's standard delivery model. The program will use interdisciplinary teams to provide comprehensive medical and social services including, at minimum, primary care and long-term care services. Organizations that offer the program must assign participants to an interdisciplinary team. The team will then assess participant needs and develop a care plan with the assistance of the individual.
