All health maintenance organizations must provide subscribers and enrollees with a list of providers at the start of enrollment. If a health maintenance organization makes material changes in its operation that will directly affect subscribers, the health maintenance organization must inform the subscribers within 30 days. If a primary care provider’s contract is terminated, the health maintenance organization must inform all subscribers who used that primary care provider.
