Requires the Department of Insurance to make an external review request form available on the Department’s website. Requires covered persons to include an authorization to disclose health information form when requesting an external review. Prohibits the Department from acting on an external review request until they receive this authorization. Permits covered persons to designate another person to serve at their authorized representative during for the external review.
