Identifies the disclosures of identifying health information the Department of Health and Welfare must document (e.g., disclosures for public health activities, pursuant to state or federal law, research, etc.). Requires the documentation to include the date of disclosure, the recipient, the type of information disclosed, and the purpose for the disclosure. Permits individuals or their authorized representatives to request a report from the Department about disclosures of their information. Requires the Department to suspend reports of disclosures if requested by a federal, state, or local government entity that is conducting an oversight or licensing investigation. Limits verbal requests to suspend reporting to 30 days, but permits renewal of the suspension via written request. Requires the Department to retain disclosure records for six years.
