Provides that individuals—or their legal or information representatives— who sign a Department of Health and Welfare application consent to the Department using, gathering, and disclosing information as necessary to provide benefits or services to that individual. Makes the failure to consent on the application form grounds for denying a service. Clarifies that consent by informal representatives is only valid when specifically stated in the Chapter 16.05.01 rules.
