Requires hospitals to conduct patient assessments and develop an individualized care plan that identifies goals, addresses pain management, and coordinates care with other services. Prohibits the provision of medications or treatments unless directed verbally or in writing by a physician or other authorized practitioner. Requires verbal orders to be authenticated and verified. Requires hospitals to develop discharge plans for each patient and to discuss these plans with the patient or their representative prior to discharge. Identifies elements that these discharge plans must address (e.g., an evaluation of post discharge care needs, self-care instructions, etc.).
