“Implementation of the long-term community diversion pilot projects under the Long-Term Care Community Diversion Pilot Project Act”
The Department of Elderly Affairs must work with the Agency for Health Care Administration to implement long-term care community diversion pilot projects. The department must select projects whose design and providers demonstrate capacity to place participants in the least restrictive appropriate care setting. The department is responsible for enrolling prospective participants in community diversion pilot projects.
The long-term care community diversion pilot projects are required to:
- Provide services for participants that are of sufficient quality, quantity, type, and duration to prevent or delay nursing facility placement;
- Integrate acute and long-term care services;
- Encourage individuals, families, and communities to plan for their long-term care needs; and
- Provide skilled and intermediate nursing facility care for participants who cannot be adequately cared for in noninstitutional settings.
