Dentists of record are responsible for ensuring that patient records include treatment records for patients treated in dental offices, clinics, or by other providers that offer dental services. Arizona defines dentist of record as a dentist responsible for examining, diagnosing, and designing a patient treatment plan. Patients must consent to a change in their treatment plan. Dentists of record must find a replacement dentist of record upon leaving their place of practice or the profession and identify this new dentist of record in each patient record.
