Collaborative pharmacy practice agreements - pharmacist requirements
Establishes qualifications for pharmacists to enter into collaborative pharmacy practice agreements with physicians (e.g., pharmacist must be licensed in Colorado, have professional liability insurance, establish a process for documenting changes in patient medical records and communicating with providers, etc.). Permits pharmacists to enter into these agreements with any practitioner pursuant to a statewide protocol. Absent such protocol, pharmacists may only enter in agreements with a provider with whom the patient has a relationship.
