Collaborative Drug Therapy Management
Authorizes pharmacists to provide collaborative drug therapy management pursuant to a written protocol. Specifies the information that must be included in this protocol when done in a community setting (e.g., names of pharmacists authorized to provide the drug therapy management services, provider signatures, drug orders, patient activities, etc.). Requires the pharmacist or provider that starts the protocol to obtain the patient’s written consent to the drug therapy management and to provide a copy of this consent to the other participating pharmacies or providers within 24 hours. Requires providers to document the patient’s consent in their medical records. Authorizes drug therapy management to occur in hospital settings. Specifies the information that must be included in these protocols (e.g., names of pharmacists authorized to provide drug therapy management services, medication orders, quality assurance protocols, etc.).
