Client Confidentiality and Consents
Requires substance abuse programs to comply with federal regulations regarding the confidentiality of client health information and records. Requires clients to sign a form stating that the program explained the confidentiality requirements to them and they understand what information is confidential, what information may or may not be disclosed, and when such disclosure may occur. Requires programs to have clients or guardians sign a treatment consent form, an acknowledgement of the program’s policies and procedures, and a client rights form. Requires programs too establish a procedure for handling client grievances and for clients or their guardians to sign a form stating that they understanding this procedure.
