Authorizations
Defines the form and content of a valid authorization to disclose nonpublic personal health information. Includes, among other items, a requirement to identify the person about whom the information pertains, a description of the information disclosed, and a description of how the information will be used. Permits individuals to revoke their authorization at any time. Requires licensed insurance carriers to retain a copy of the authorization in their records.
