“Release of medical records and payment records to patients and health care decision makers; definition”
Health care providers must honor the written request of a patient or their health care decision maker to access or copy the patient’s medical or payment records. Arizona permits providers to deny access to medical records in limited circumstances including when the provider believes that access to the records could endanger the life or safety or the patient or other persons, the records contain references to another individual who could be harmed by the access, or the records contain information regarding research and the patient consented to not having access to this information. Health care providers must explain their decision to deny access to the patient or their decision maker in writing and document the decision in the patient’s records.
