An approved risk-assuming preferred provider must annual file with the secretary and the commissioner of insurance a report including the following:
- A copy of an annual financial statement;
- A description of results of its quality assurance activities;
- A summary of the number of covered persons in the preferred provider organization; and
- A summary of utilization experience of the organization.
