“Access to records”
The department of health services and local health departments may access provider and facility medical or laboratory records when investigating reportable communicable diseases. The person conducting an investigation may gain access to such records by submitting a written request to the provider or facility that identifies the person and disease under investigation. Arizona limits an investigator’s access to medical and laboratory records to information related to the diagnosis, treatment, and control of the disease that is under investigation.
