“Hospital policies, procedures and reporting related to all adverse events and near misses under the Agency of Human Services regulations”
Each hospital must establish an internal reporting system and develop policies and procedures to identify, track, and analyze adverse events and near misses to improve patient safety. A root cause analysis must be conducted at each adverse event. A corrective action plan must be developed and implemented for each adverse event. Each hospital must develop and implement policies and procedures for disclosures to patients or a patient’s family relating to the adverse events that cause death or serious bodily injury. Each hospital must submit an initial report, root cause analysis, and corrective action plan for each adverse event to the patient safety surveillance and improvement system.
