“Physician Reporting Requirements”
Any physician who makes a diagnosis of a congenital anomaly prenatally, at delivery or between birth and up to three years of age shall report such diagnosis to the department. Although health care facilities and physician practice groups may act as agents for individual physicians by submitting the report directly to the department, the reporting requirement remains the responsibility of the physician who makes the diagnosis. All diagnoses shall be classified and coded as directed by the commissioner.
